Few treatments have a worse reputation than the root canal – yet much of that worry is based on outdated information. In reality, modern root canal treatment is a routine, comfortable procedure that saves millions of teeth every year. If you have been told you need one, here is exactly what to expect.

What is a root canal?

Inside every tooth is a soft core called the pulp, which contains the nerve and blood supply. It runs from the crown of the tooth down through narrow channels in the roots – the root canals. If the pulp becomes infected or inflamed, through deep decay, a crack or injury, it can be very painful and the infection can spread. Root canal treatment removes the damaged pulp, cleans and disinfects the canals, and seals the tooth so it can be saved rather than extracted.

Signs you may need root canal treatment

  • Persistent or throbbing toothache, sometimes worse at night.
  • Lingering sensitivity to hot or cold, even after the trigger is removed.
  • Pain when biting or chewing.
  • A darkening tooth, or a small “pimple” on the gum nearby.
  • Swelling or tenderness in the gum or face.

Sometimes there are no symptoms at all, and the problem is only found on a routine X-ray – another good reason to keep up regular check-ups.

Does a root canal hurt?

This is the question we hear most. The treatment itself is carried out under local anaesthetic, so the tooth and surrounding area are completely numb – most patients say it feels much like having a filling. In fact, because the procedure relieves the infection causing your pain, the lasting effect is to make you far more comfortable, not less.

What happens during treatment

  1. Numbing the tooth – we make sure the area is fully anaesthetised before we begin.
  2. Accessing the pulp – a small opening is made in the top of the tooth.
  3. Cleaning the canals – the infected pulp is removed and the canals are carefully cleaned and shaped. Back teeth such as molars have more canals, so these can take a little longer.
  4. Sealing the tooth – the canals are filled and sealed to prevent further infection.
  5. Restoring the tooth – a filling or, often, a crown is placed to protect the tooth and restore full strength.

Treatment may be completed in one or two appointments, depending on the tooth and the level of infection.

Caring for your tooth afterwards

It is normal for the tooth to feel a little tender for a few days, which usually settles with ordinary pain relief. A root-canal-treated tooth can last for many years – often a lifetime – with good care. Brush and floss as normal, keep up your regular check-ups, and if a crown has been recommended, it is well worth having, as it greatly reduces the risk of the tooth fracturing later.
